For your convenience, CIEE has compiled the forms needed to apply for our programs, as well as other useful materials. Your CIEE international representative may have additional instructions and other documents for you. All application materials should be completed in English and submitted to your CIEE international representative.

J-2 Dependent Application - Spouse
This form is for spouses who accompany J-1 participants to the U.S. using the J-2 visa. It can be submitted with your program application or later. For more information, please read our page on J-2 Visas for Dependents.

J-2 Dependent Application - Child
This form is for children who accompany J-1 participants to the U.S. using the J-2 visa. It can be submitted with your program application or later. For more information, please read our page on J-2 Visas for Dependents.

Application for Program Extension
This form is required for program extensions. You will also need to complete a new online DS-7002 with your host organization. For more information, please read our page on Extending, Changing, or Ending Your Program Early

Guarantor Letter 
This letter is optional and can be used to prove financial stability during your program.

English Language Ability Form 
This form is optional and can be used as proof of your English language skills.
