The DS-7002

Completing the DS-7002 is a key part of the process. We encourage host organizations and participants to discuss their training plans in detail to ensure a successful experience. Our trained and experienced staff are available to help!

DS-7002 for Trainees

If your organization will be hosting a Trainee, you will have to divide your placement plan into multiple phases—each with a specific training objective. Please use the outline below when filling out the DS-7002.

Cover Page

  • Participant Information: This section pertains to the Trainee and may be filled out by you or the applicant. Please complete the "Training/Internship Date" section.
  • Site of Activity Information: Please complete this section in its entirety.
    • Hours per Week: Trainees are required to train for at least 32 hours per week.
    • Employer ID Number: Your organization must have an Employer ID number before hosting.
    • DUNS Number: this field is optional and may be left blank if you do not have your company’s Dun & Bradstreet Number available, or if your company does not have one.
  • All fields relating to the Sponsor should be left blank. These are for CIEE use only.

Phase Page (pages 2-7)

The Phase page describes the nature of the training your Trainee will receive. All sections should be filled out with as much detail as possible.

  • Brief Description of Trainee/Intern’s Role for this Program or Phase: Provide a description of the trainee’s function in your organization during this program. What are the responsibilities associated with this position?
  • Methodology of Training and Chronology/Syllabus for this Phase: Include a description of the methods of training that will be employed during this phase in order advance the trainee’s knowledge and skills. Offer a detailed description of the types of day-to-day activities the trainee will be expected to participate in and assist with during this program or phase. Try to list the tasks chronologically so that the overall structure of the program can be seen.
  • Specific Goals and Objective for this Program or Phase: Describe the goals and objectives that will guide the training program. Include information about what you hope to impart upon the trainee and what you expect he/she will contribute to your organization.
  • Knowledge, Skills, or Techniques to be Imparted During this Program or During this Phase: Explain the kinds of knowledge, skills, and techniques you expect the trainee to gain during his/her time with your organization based on your understanding of the applicant's current skill set.
  • Methods of Evaluation and the Frequency of Supervision for this Program or for this Phase: Include information about how you expect to supervise and track the progress of the trainee on both a day-to-day and long-term basis and the level of supervision associated with this internship.

The last page of the DS-7002 is a CIEE-generated page that includes information required by the Department of State. You must complete this page in its entirety, except in those cases where the information does not apply. Your organization’s eligibility for hosting is determined by the information provided here.

  • Workers’ Compensation: Host organizations are required to have a workers’ compensation policy in place prior to participating in the program.
  • Annual Revenue: Only academic institutions and government entities are not required to provide annual revenue details. Your organization’s annual revenue will help us determine whether or not a site visit is necessary.
